Baptism Interest Form

If you are interested in learning more about baptism and/or getting baptized, please complete the form below. You will receive an email response with resources that we encourage you to watch/read.

Someone from our baptism team will follow up to answer any questions you have and provide information about our next baptism dates.

*Please number your responses so we can match them up to each question. We will share and celebrate your responses with the church the morning you are baptized!

Acts 2:38 says, "Repent and be baptized every one of you in the name of Jesus Christ for the forgiveness of your sins, and you will receive the gift of the Holy Spirit." Have you chosen to repent/turn from your sins? 

In Luke 9:23, Jesus said, “If anyone would come after me, let him deny himself and take up his cross daily and follow me." What do you think you will have to give up/deny yourself of? What do you think will be hard/a daily cross? Are you willing to walk this out for Jesus?

Is there another follower of Jesus who has played a role in your decision to follow Jesus and be baptized that you would like to baptize you? If so, please include their name, email address, and telephone number. Also, please let them know that you would like them to baptize you and that someone from the church will follow up with them.
